Instructions for Nurse Practitioners
There are 3 steps required to complete the training requirements for the Arizona State Board of Nursing:
- Complete and submit the required Confidentiality Agreement forms provided by the Arizona Department of Health Services.
- Complete the e-learning training provided by the Arizona Department of Health Services.
- Complete the Death Certificate Education Form under Other Applications at the AZ Nurse Portal.
